The forms that I have attached need to be completed before being submitted to Human Resources, for the COVID for Volunteers PDF, only the last page of the packet needs to be signed and returned. A tip for the packet is email the pdf to the volunteer and print the last page for the signature rather than printing 30 pages for each volunteer. The volunteers should be entered into the Raptor System as a form of background check if they have a valid government issued I.D., if they do not have a valid government issued I.D., then the Megan's Law Background Check form must be completed and the form must be on file with the school site.
TB testing is required for all volunteers. The District will provide TB testing at no cost to the volunteer, in the form of an online Adult Tuberculosis (TB) Risk Assessment Questionnaire. To complete the questionnaire please have the volunteer visit the Human Resources Department or they may complete the questionnaire at a school site on a District computer at the school site. Fingerprints are required if the volunteer(s) will be volunteering for four (4) or more hours per week or will be attending an overnight field trip. If they have already been fingerprinted with the district, please contact Human Resources to confirm that we have the fingerprint report on file. If the volunteer(s) is attending a ONE (1) day field trip, fingerprinting is not required. The volunteer(s) would need to be approved by the site using the Raptor System.
Volunteer needs to provide proof of full COVID-19 vaccination, please scan proof with the volunteer packet. If they DO NOT have proof of vaccination, they will need to undergo weekly testing before volunteering at the site. This will be at the parents' expense.
All the documents need to be scanned and emailed to Mekesha Bush, Human Resources Clerk III. Please include George Loya in the email as he processes the volunteer's COVID vaccinations, tests, and Prevention Program Acknowledgment and clears the volunteers for Risk Management as well. Please note that the process can take up to a few weeks depending on fingerprints and TB clearances.
